The plans dated 05/07/08 and revised 5/28/08 for the captioned project have been reviewed for compliance with the
provisions of the 2007 Ohio Building Code (OBC). The review was based upon the following criteria:

Notes:
The construction documents are sealed by Dan Herbert Marsh., Ohio registered architect #7319 and Jorge R. Rossi,
Ohio registered engineer #E42690 to comply with the requirements of the OBC Section 106.3.4.1 and have been
reviewed, therefore, in accord with OBC Section 104.1.
OBC 106.3.1 Approval of construction documents. When the construction documents have been determined to
conform to the applicable provisions of the rules of the Board in accordance with the procedures set forth in Section
105.3.1, the building official shall endorse or stamp such plans as approved. One set of con.struction documents shall
be kept by the building official. The other set(s) shall be returned to the applicant, kept at the work site and shall be
open for inspection by the building official or the building official's designated representative.
The Drawings are APPROVED.
The following items are reguired for code compliance, but are not necessarily covered in detail in the construction
documents. This list is a reminder to the design professional and contractors of issues, which are to be
satisfactorily dealt with in the field:
Item A OBC 106.3.1 Approval of construction documents. When the construction documents have been
determined to conform to the applicable provisions of the rules of the Board in accordance with the
procedures set forth in Section 1053.1, the building official shall endorse or stamp such plans as
approved. One set of construction documents shall be kept by the building official. The other set(s)
shall be returned to the applicant, kept at the work site and shall be open for inspection by the building
official or the building official's designated representative.
Item B OBC 105.7.2 Posting. The certificate of plan approval shall be posted in a conspicuous location
outside of the building and in the front part of the premises on which is, or will be the approved
building or equipment to which the approved plans relate. The owner and the contractor shall, so far as
possible, preserve and keep the certificate posted until the completion of the work to which the
approved plans relate
Item C OBC 106.4 Amended construction documents. If substantive changes to the building are
contemplated after first document submission, or during construction, those changes must be submitted
to the building official for review and approval prior to those changes being executed. The building
official may waive this requirement in the instance of an emergency repair, or similar instance.
Item D OBC 109.1 General. Construction or work for which an approval is required shall be subject to
inspection by the building official and such construction or work shall remain accessible and exposed
for inspection purposes until approved.

? This includes firestopping and draftstopping (Chapter 7 OBC), mechanical work; piping, ducts and
systems (Chapter 3 OMC), structural members and connections (Chapter 16 OBC), and electrical
work (Chapter 27 OBC). All systems and elements covered 6y code are to be inspected and approved
before being covered.
Item E OBC 1103.1 Where required. Buildings and structures, temporary or permanent, including their
associated sites and facilities, shall be accessible to persons with physical disabilities.
Item F OBC 1103.2.1 Specific requirements. Accessibility is required in buildings and facilities, or portions
thereof, to the extent indicated in ADAAG Section 4.1 and this chapter.
Item G All electrical will comply with the requirements of Article 27 OBC and the National Electrical Code,
NFPA 70, OBC approved edition and is subject to the approval of the electrical fieid inspector.
Item H The existing electrical conductors, if removed, abandoned, or altered, shall be accomplished to the
electrical field inspector's approval. Electrical work found not to meet the code's standards shall be
corrected to the electrical field inspector's approval.
The start of construction indicates acceptance of all of the conditions listed above. This review does not preclude
the necessity to conform to provisions which may have been omitted or overlooked in the review process, but which
are requirements of the code. Ultimate responsibility for legal compliance with the Standards of Safety rests with
the registered design professional, the tenant and the owner. Plan Approval will be valid only upon receipt by the
City of Dublin, Division of Building Standards, of one signed copy of this Plan Approval to the submitted plans.

provisions of the Ohio Building Code (2007). The review was based upon the following criteria:

The construction documents are sealed by Dan Herbert Marsh., Ohio registered architect #7319 and Jorge
R. Rossi, Ohio registered engineer #E42690 to comply with the requirements of the OBC Section
106.3.4.1 and have been reviewed, therefore, in accard with OBC Section 104.1.
OBC 106.3 Examination of documents. The building official shall examine or cause to be examined
the accompanying construction documents and shall ascertain by such examinations whether the
construction indicated and described is in accordance with the requirements of this code.
The construction documents, which have been submitted, do not allow the City of Dublin to issue a
permit to begin construction. The following item(s) shall be brought into full compliance with the
requirements of the OBC prior to the issuance of any plan approval.
Item 1 106.1.1 Information on construction documents. Construction documents shall be
dimensioned and drawn upon suitable material. Electronic media documents are permitted to
be submitted when approved by the building official. Construction documents shall be
coordinated and of sufficient clarity to indicate the location, nature and extent of the work
proposed and show in detail that it will conform to the provisions of this code. Construction
drawings shall include information necessary to deternune compliance with the building,
mechanical, plumbing and fire protection codes such as:
a. 9. System descriptions. Complete description of the rrtechanical and electrical
systems, including: materials, routing, and sizes of all piping; location and type of
plumbing fixtures artd equipment; plumbing schematics and isometrics; materials,
routing and sizes of all ductwork; location and type of heating, ventilation, air
conditioning and other mechanical equipment and all lighting and power equipment.
i. Please identify the size of the existing HVAC System and the required CFM for
each Furnace Unit. Srrcoke detectors are required by OMC 606.2.1 in return air
systems exceeding 2,000 CFM.
Item 2 Accessible patient counter is required by ADAAG 7.2 to be 3'-0" wide, minimum.

Item 3 Health Care Grade wiring is required by NEC 517.13 for all patient care areas. MC Cable is
not permitted by the NEC for Patient Care Areas.

Item 4 NEC 517.25 requires that emergency lighting be provided for each patient treatment location.

Item 5 Lighting and Power Compliance Certificate (90.1 (2004) Standard) Section 3(5) requires
occupancy sensor controls for conference/meeting rooms and employee lunch and break
rooms.
a. Provide occupancy sensor controls for the Staff Lounge and the Consultation rooms.
Item 6 The Washington Township Fire Marshal has requested an opportunity to provide input to the
Dublin building department on issues relating to fire protection. The building official has
evaluated the Fire Marshal's comments related to the fire protection provisions of the OBC
and incorporates those comments, by reference, into this Phased Plan Approval and
Correction Request.
a. Please include resolution of any of the matters of the Fire Marshal's correspondence to
Building Standards in your reply to this Correction Request.
This review is based upon the information contained within the submitted drawings and related
documents. Once corrections are made, submit four (4) new and complete sets of plans to the City of
Dublin, Division of Building Standards with any revisions clouded. Please also provide a letter that
identifies the action taken in rsponse to any of the Items contained in this Correction Request.

is to summarize all revisions required for zoning compliance only. Please review the
contents of this letter with all plan review comments indicated on the plan set labeled
"Z/E." All revisions must be coardinated with any applicable plan review comments
made separately by the Division of Building Standards, the Office of the City Engineer,
and/or the Washington Township Fire Department. Please return all marked alans

General Comments
Site modifications listed below will first require Planning and Zoning Commission
review and approval of an Amended Final Development Plan. Please contact a planner
in Land Use and Long Range Planning at (614) 410-4600 for additional information
related to this required application and public hearing process.
Specific Comments
• The change of use from office to medical (dental) office requires an increase in
parking from one space per 250 square feet to one space per 200 square feet.
The combined parking demand as required by code the dental practice and the
bank is a minimum of 36 spaces plus one space per bank employee. According
to the site plan only thirty six spaces are shown on site.
• 27% of the existing 8757 square foot building is affected by the construction of
the proposed dental office. Section 153.132(D) of the Landscape Code of the
City of Dublin requires that the entire site be brought into conformance with the
landscape code. Please contact Brian martin at (614) 410-4660 regarding this
provision.
• The site plan does not reflect the removal of the former Fed/Ex building to the
west. The site plan must be revised to reflect all current conditions that are to
remain and all new requirements related to the site design.

• The parking code requires a parking lot of 36 parking spaces to provide a
minimum of two handicap accessible parking spaces, one of which must be van
accessible. Only one handicap space is currently shown on the site plan.
Additional handicap parking must be added and all required ramps, signs and
other ADAAG requirements must be shown on the permit sets.

The table should identify the square footage of both tenant spaces, the required parking
ratio for each (1/200 SF for the dentist, and one/250 SF plus 1/employee for the bank.)
The table should also confirm that the bank will have no more than seven employees on
site. It should also list the number of required and provided handicap spaces on site.
Second, the property owner must provide a letter to the City of Dublin that confirms the site
will be brought into conformance with the provisions of the City of Dublin Landscape Code as
required by Code Section 153.132(B).
Please Note: Until the two items described above are completed the building permit application
is considered to be disapproved but we will hold all plans on file. The pernut application
status will be changed to an approval as soon as the items described above are addressed.
Brian Martin of the City of Dublin is working with the property owner to establish the
requirements of the written agreement and to arrange delivery of the letter. The property owner
will be allowed to break the required landscape installation into phases with a final completion
date that is acceptable to the City of Dublin. Following the City's receipt and acceptance of the
letter, Dublin Metro Dental Group will not be required to submit landscape plans for review or
install any landscape material as part of the building permit to be issued for their tenant
improvements. Occupancy for the dental offices will not be contingent upon satisfaction of
the landscape Code requirements provided the property owner commits to full
compliance with the Landscape Code and a specific time frame for completion of the
work.

The plans, dated 5/6/08 for the captioned project have been reviewed for compliance with the provisions of the 2007
Ohio Building Code (OBC). The review was based upon the following criteria:

Occupancy: 50 @ 1:100
Notes: 1. Building is equipped with fully automatic sprinkler system. Sprinkler
Application # 08-200527 is approved.
2. The building is provided with an enunciated automatic rire protective
signaling system with audible and visual alarms in all public spaces.
Detection Application # 08-200645 is approved.
3. The return air plenum is protected by smoke detectors at each fan coil unit.
The architectural documents are sealed by Jennifer Powell Carney, Ohio registered architect #10748, the plumbing,
mechanical and electrical by Bruce Vetter, Ohio registered engineer #63372 to comply with the requirements of the
OBC Section 106.3.41 and have been reviewed, therefore, in accord with OBC Sectio? 104.1.
OBC 106.3.1 Approval of construction documents. When the construction documents have been determined to
conform to the applicable provisions of the rules of the Board in accordance with the procedures set forth in Section
105.3.1, the building official shall endorse or stamp such plans as approved. One set of construction documents shall
be kept by the building official. The other set(s) shall be returned to the applicant, kept at the work site and shall be
open for inspection by the building official or the building official's designated representative.
The Drawings are APPROVED.
The following items are required for code compliance, but are not necessarily covered in detail in the construction
documents. This list is a reminder to the design professional and contractors of issues, which are to be
satisfactorily dealt with in the field:
Item A OBC 106.3.1 Approval of construction documents. When the construction documents have been
determined to conform to the applicable provisions of the rules of the Board in accordance with the
procedures set forth in Section 105.3.1, the building official shall endorse or stamp such plans as
approved. One set of construction documents shall be kept by the building official. The other set(s)
shall be returned to the applicant, kept at the work site and shall be open for inspection by the building
official or the building official's designated representative.
Item B OBC 105.7.2 Posting. The certificate of plan approval shall be posted in a conspicuous location
outside of the building and in the front part of the premises on which is, or will be the approved
building or equipment to which the approved plans relate. The owner and the contractor shall, so far as
possible, preserve and keep the certificate posted until the completion of the work to which the
approved plans relate
Item C OBC 106.4 Amended construction documents. If substantive changes to the building are
contemplated after first document submission, or during construction, those changes must be submitted

to the building official for review and approval prior to those changes being executed. The building
official may waive this requirement in the instance of an emergency repair, or similar instance.
Item D OBC 109.1 General. Construction or work for which an approval is required shall be subject to
inspection by the building official and such construction or wark shall remain accessible and exposed
for inspection purposes until approved.
This includes firestopping and draftstopping (Chapter 7 OBC), mechanical work; piping, ducts and
systems (Chapter 3 OMC), structural members and connections (Chapter 16 OBC), and electrical
work (Chapter 27 OBC). All systems and elements covered by code are to be inspected and approved
before being covered.
Item E OBC 1103.1 Where required. Buildings and structures, temporary or permanent, including their
associated sites and facilities, shall be accessible to persons with physical disabilities.
Item F OBC 1103.2.1 Specific requirements. Accessibility is required in buildings and facilities, or portions
thereof, to the extent indicated in ADAAG Section 4.1 and this chapter.
Item G All electrical will comply with the requirements of Article 27 OBC and the National Electrical Code,
NFPA 70, OBC approved edition and is subject to the approval of the electrical field inspector.
Item H The existing electrical conductors, if removed, abandoned, or altered, shall be accomplished to the
electrical field inspector's approval. Electrical work found not to meet the code's standards shall be
conected to the electrical field inspector's approval.
The start of construction indicates acceptance of all of the conditions listed above. This review does not preclude
the necessity to conform to provisions which may have been omitted or overlooked in the review process, but which
are requirements of the code. Ultimate responsibility for legal compliance with the Standards of Safety rests with
the registered design professional, the tenant and the owner. Phased Plan Approval will be valid only upon receipt
by the City of Dublin, Division of Building Standards, of one signed copy of this Phased Plan Approval to the
submitted plans.

OBC 107.3.3 Phased approval. The building official shall issue an approval for the construction of foundations or
any other part of a building or structure before the construction documents for the whole building or structure have
been submitted, provided that adequate information and detailed statements have been filed complying with
pertinent requirements of this code. The holder of such approval for the foundation or other parts of a building or
structure shall proceed at the holder's own risk with the building operation and without assurance that an approval
for the enCire structure will be granted. Such approvals shall be issued for various stages in the sequence of
construction provided that all information and data reguired by the code for that portion of the building or structure
has been submitted. The holder of a phased p[an approval may proceed only to the point for which approval has
been given
This Phased Plan Approval is For PRE WALLBOARD (Interior Framing without wallboard) AND ROUGH

issuance of any further Plan Approvals.
Item 1 Fire Suppression Drawings shall be submitted for review; contain all information required by NFPA
13-2007 Chapter 14; and be approved before any equipment is installed or remodeled. A separate
Permit from the city of Dublin is required.
Item 2 Fire Alarm Drawings shall be submitted for review; contain all information required by NFPA 72-
2007 4.5.1.1 and explained in Annex A4.5.1.1; and be approved before any equipment is installed or
remodeled. A separate Permit from the city of Dublin is required.
Item 3 ADAAG 7.2 (2) Sales and Service Counters At ticketing counters, teller stations in a bank,
registration counters in hotels and motels, box office ticket counters, and other counters that may not
have a cash register but at which goods or services are sold or distributed either (i) a portion of the
main counter a minimum of 36 in in length shall be provided with a maximum of 36 in in height; or (ii)
an auxiliary counter with a maximum height of 36" in close proximity to the counter shall be provided;

Item 4 Provide details for the gypsum wallboard accent ceiling in the Waiting Room and coordinate with the
suppression drawings to be submitted for review.
Item 5 The Washington Township Fire Marshal has requested an opportunity to provide input to the Dublin
building department on issues relating to fire protection. The building official has evaluated the Fire
Marshal's comments related to the fire protection provisions of the OBC and incorporates those
comments, by reference, into this Phased Plan Approval and Correction Request.
Please reply to this request for additional information with a letter indicating your response to each item of this
request and that of the Washington Township's Fire Department comments. Once the requested information is
available, submit three (3) copies of onlv the additional information requested to the City of Dublin, Division of
Building Standards for further review.

Item A OBC 106.3.1 Approval of construction documents. When the construction documents have been
determined to conform to the applicable provisions of the rules of the Board in accordance with the
procedures set forth in Section 105.3.1, the building official shall endorse or stamp such plans as
approved. One set of construction documents shall be kept by the building official. The other set(s)
shall be returned to the applicant, kept at the work site and shall be open for inspection by the building
official or the building official's designated representative.
Item B OBC 105.7.2 Posting. The certificate of plan approval shall be posted in a conspicuous location
outside of the building and in the front part of the premises on which is, or will be the approved
building or equipment to which the approved plans relate. The owner and the contractor shall, so far as
possible, preserve and keep the certificate posted until the completion of the work to which the
approved plans relate
Item C OBC 106.4 Amended construction documents. If substantive changes to the building are
contemplated after first document submission, ar during construction, those changes must be submitted
to the building official for review and approval prior to those changes being executed. The building
official may waive this requirement in the instance of an emergency repair, or similar instance.
Item D OBC 109.1 General. Construction or work for which an approval is required shall be subject to
inspection by the building official and such construction or work shall remain accessible and exposed
for inspection purposes until approved.
This includes firestopping and draftstopping (Chapter 7 OBC), mechanical work; piping, ducts and
systems (Chapter 3 OMC), structural members and connections (Chapter 16 OBC), and electrical
work (Chapter 27 OBC). All systems and elements covered by code are to be inspected and approved
before being covered.
Item E OBC 1103.1 Where required. Buildings and structures, temporary or permanent, including their
associated sites and facilities, shall be accessible to persons with physical disabilities.
Item F OBC 1103.2.1 Specific requirements. Accessibility is required in buildings and facilities, or portions
thereof, to the extent indicated in ADAAG Section 4.1 and this chapter.
Item G All electrical will comply with the requirements of Article 27 OBC and the National Electrical Code,
NFPA 70, OBC approved edition and is subject to the approval of the electrical field inspector.
Item H The existing electrical conductors, if removed, abandoned, or altered, shall be accomplished to the
electrical field inspector's approval. Electrical work found not to meet the code's standards shall be
corrected to the electrical field inspector's approval.
The start of construction indicates acceptance of all of the conditions listed above. This review does not preclude
the necessity to conform to provisions which may have been omitted or overlooked in the review process, but which
are requirements of the code. Ultimate responsibility for legal compliance with the Standards of Safety rests with

Mr. Harpham:
We have reviewed the construction documents for the referenced Interior Improvements, with as submitted to the Division of
Building Standards May 9, 2008, for matters of concem to the Washington Township Fire Department regarding rules relating
to the 2007 Ohio Building Code (OBC) and the fire prevention and the Dublin Fire Code(DFC) based upon the 2000
Intemarional Fire Code.
We are able to recommend approval of the plans submitted with the following conditions:
OBC 106.1.1 Information on construction documents. Because the construction documents do not have enough
information for a complete plan review, this correction letter is a request for missing information and contains a review
only of the items submitted. A complete review will be performed upon receipt of the required information. This mav
result in additional items not contained in this conection letter; Section 106 OBC.
1.1 Please explain how the drywall ceilingAccent is to 8e handled at the WaitingArea in this Tenant space?
Currently, SheetA1.02 calls for the drywall accent to be installed at 8'-0"AFFwhile the adjacent lay-in
acoustical ceiling system is noted to be installed at 8'-10"t1FF. How will the change in ceilfng elevations be
handled? No soffit or bulkhead details were provided for our review. If this drywall ceiling is to be handled as
a separate plane ' suspended below the lay-in ceiling, sprinkler protection may be required above AND below
this ceiling accent. Please verify.

2. OBC 106.1.1.1 Fire protecNon system drawings. Drawings for the fire protection system(s) shall be submitted to
indicate conformance with this code and the construction documents and shall be approved prior to the start of system
installation. Drawings shall contain all information as required by the referenced installation standards in OBC Ch 9.
a. When they are made available, submit for approval, prior to the stan of system(s) installation, complete
information regarding the fire protection systems or the alteration of the system(s) information required by the
2007 NFPA 13, Chapter 22, Plans and Calculations for Fire Suppression Systems, and the device
specifrcations, wiring diagrams, battery calculations, and floor plans required by NFPA 72 (2007) §A4.5.f'or
any revisions to the existing Fire Alarm/Detection Systems.
b. Fire protection system drawings shall be sealed by a design professional, registered in the state of Ohio or be
signed and certification number listed thereon, by an indivedual certified in accordance with Section 105.3.1.4
of the OBC.
c. Separate permits are required for the Suppression and Alarm systems.
