Hereinafter lrnown as the Owner/Occupant, and the City of Dublin, hereinafter known as the City, who
mutually agree and covenant as follows:
The Project Manager/General Contractor, who is constructing a new premises at:

Building Permit No. C6- ;lc? e 1-1 7 y , has applied for a Certificate of Occupancy.
2. The premises, as of the date this agreement is signed, has not met all requirements of the Dublin
Codified Ordinances, hereinafter knawn as the Code, and therefore additional final inspections
cannot be approved.
3. The list of items, which is attached to and is a part of the agreement, is accepted by both the Project
Manager/General Contractor and the Owner/Occupant, documentation of those items necessary to
be completed prior to the project receiving final approval.
4. Acknowledging that the premises are not completed and that certain items, as shown, are
requirements of the Code of the City, the Owner/Occupant, requests that the City allows occupancy
of the premises and use of the property as of the date of this agreement.
5. In return for such above Conditional Approval, the Project Manager/General Contractor agrees to
complete all items as shown by (date) f 0 and further agrees to release, hold
harmless and indemnify the City and its agents, assigns and employees from any and all obligation,
liability, and/or responsibility that might arise as a result of permitting occupancy under a
Conditional Approval Agreement.
6. In return for the City allowing conditional approval, the Owner/Occupant hereby releases the City,
its agent, assigns and employees from any and all obligarions and/or responsibilities related to the
completion of the premises, and further agrees to release and hold harmless the City, its agents,
assigns and employees from any and all liability that might arise as a result of occupancy of the
premises. Further, the Qwner/Occupant accepts the responsibility for the completion of those items
shown as incomplete, and recognizes that the City may revoke the Conditional Approval

Agreement causing the premises to be vacated; and/or the City may initiate legaI action, if these
items are not completed on a timely basis, and within the schedule contained in item 5 of this
agreement. The Owner/Occupant agrees that the City or its agent shall have full access to the
property to inspect, or at iYs sole and exclusive discrerion cause to be made any improvement
necessary to bring any part of the premises into compliance with this Conditional Approva]
Agreement.
7. The Owner/Occupant and the Project Manager/General Contractor both agree that the City, by
accepting this Agreement in allowing occupancy as of the date of this Agreement has in no way
waived, forfeited or otherwise relinquished any rights and/or powers that it would have if this
Agreement were not in effect.
8. The Owner/Occupant acknQwledges that he is under na obligation whatsoever to enter into this
Agreement, but that absent this Agreement, Conditional Approval to use the property will not be
granted.
9. This Conditionai Approval Ageement will serve as the Temporary Occupancy Permit the Building
Official may issue in accordance with 4101:2-1-27(E) of the Ohio Administrative Code when the
entire scope of the building work has not been completed, but the Building Official has determined
the building can be occupied safely.

Vw Nonrefundable Conditional Approval Agreement handling fee of $300.00. This is in
addition to any surety amount listed on the Conditianal Occupancy inspection notice.
This fee will be good for the tim period noted on the Building Inspector's inspection

$300.00 handling fee will be billed for each six (b) month time period. A notice will be
sent to the Project Manager/General Contractor and Owmer/Occupant prior to the
expiration of the Conditionat Agreement. If a Conditional expires, an additional$3f}0.00
fee will be due prior to an Occupancy inspeetion being scheduled.
0 The amount listed on the Conditional Occupancy Inspection Notice in the form of a
check or an original escrow letteron letterhead of the issuing financial institution and
with the original signature, printed name, title and telephone number of the issuing agent
for the financiat institution. Commercial projects also have the option of providing a
bond or a letter of credit although addirional time wilt be required for review.
? The Chief Building Official will review the Conditional Approval Agreement within
eight business hours for residenrial and 16 busmess hours for commercial. It will be
signed after this review if it is deemed there are no safety issues involved and the above
items have been submitted to the Building Standards Division.
0 The building/home may be occupied AFTER the Chief Building Official has signed the
Conditional Approval Agreement.
Note: Residential Conditionals will be issued for sod and other exterior, weather-related items
only. Any exceptions must be pre-approved, in writing, by the Chief Building Official and
available to the Building Inspector at the rime of the inspecrion for Conditional Approval. (Write
a letter to the CBO explaining the exception you are requesting and it will be signed and retumed,
if approved.)

CONDITIONAL APPROVAL AGREEMENT
This agreement made and entered into by and between TyE (ProjB t Manager/General Contractor)
Herein after lrnown as the Project Manager/General Contractor, and f?A1?k C oIo%M v N(Ty c{WR&A

Hereinafter lrnown as the Owner/Occupant, and the City of Dublin, hereinafter known as the City, who
mutually agree and covenant as follows:
1. The Project Manager/General Contractor, who is constructing a new premises at:

Building Permit No. Gie-;l0 1 y7y , has applied for a Certificate of Occupancy.
2. The premises, as of the date this agreement is signed, has not met all requirements of the Dublin
Codified Ordinances, hereinafter lrnown as the Code, and therefore additional final inspections
cannot be approved.
3. The list of items, which is attached to and is a part of the agreement, is accepted by both the Project
Manager/General Contractor and the Owner/Occupant, documentation of those items necessary to
be completed prior to the project receiving final approval.
4. Aclrnowledging that the premises are not completed and that certain items, as shown, are
requirements of the Code of the City, the Owner/Occupant, requests that the City allows occupancy
of the premises and use of the property as of the date of this agreement.
5. In return for such above Conditional Approval, the Project Manager/General Contractor agrees to
complete all items as shown by (date) and further agrees to release, hold
harmless and indemnify the City and its agents, assigns and employees from any and all obligarion,
liability, and/or responsibility that might arise as a result of permitting occupancy under a
Conditional Approval Agreement.
6. In return for the City allowing conditional approval, the Owner/Occupant hereby releases the City,
its agent, assigns and employees from any and all obligarions and/or responsibilities related to the
completion of the premises, and further agrees to release and hold harmless the City, its agents,
assigns and employees from any and all liability that might arise as a result of occupancy of the
premises. Further, the Owner/Occupant accepts the responsibility for the completion of those items
shown as incomplete, and recognizes that the City may revoke the Conditional Approval

Agreement causing the premises to be vacated; and/or the City may initiate legal action, if these
items are not completed on a timely basis, and within the schedule contained in item 5 of this
agreement. The Owner/Occupant agrees that the City or its agent shall have full access to the
property to inspect, or at it's sole and exclusive discrerion cause to be made any improvement
necessary to bring any part of the premises into compliance with this Conditional Approval
Agreement.
7. The Owner/Occupant and the Project Manager/General Contractor both agree that the City, by
accepting this Agreement in allowing occupancy as of the date of this Agreement has in no way
waived, forfeited or otherwise relinquished any rights and/or powers that it would have if this
Agreement were not in effect.
8. The Owner/Occupant aclmowledges that he is under no obligation whatsoever to enter into this
Agreement, but that absent this Agreement, Conditional Approval to use the property will not be
granted.
9. This Conditional ApprovaI Agreement will serve as the Temporary Occupancy Permit the Building
Official may issue in accordance with 4101:2-1-27(E) of the Ohio Administrative Code when the
entire scope of the building work has not been completed, but the Building Official has determined
the building can be occupied safely.

Thank you for your voicemail message today again confirming that with your recent resubmittal you
included a written request for a"foundation only" building permit for this project. I have brought your
request to the attention of the Review Servcies Team and our Chief Building Official.
Last Friday I relayed to you that requesting a"foundation only" permit does not speed up our unified and
coordinated review process which involves five areas of plan review (Building, Fire, Engineering,
Landscape, and Zoning).
Though it is still too early to tell, as review of the resubmittal progresses, it is possible we may find it
sufficiently compliant to be eligible for a"full" permit. Should this be the case, we hope you would be
willing to accept a"full" permit, instead of the "foundation only' you have requested.
Please let me know if you have additional questions or desire further explanation of our procedures.

Dear Mr. Reker:
This is a courtesy notification of the plan review status for this commercial
permit. Review Services has completed review of submitted materials and at this
time, the overall review status is generally favorable. You may wish to forward a
copy of this notice to the developer/property owner.
Site engineering mylars were recently requested from the project's civil engineer
with an itemization of any remaining review comments to be addressed. Once we
have received and approved the mylars, the Team plan review will be formally
upgraded to approved. (iJntil then, the overall review outcome may appear as
disapproved in our plan tracking software.)
When received, the mylars will be checked for compliance, and routed internally
for signature by the Dublin City Engineer and the Director of Land Use & Long
Range Planning. Copies of approved mylars will then be assembled into
approved pertnit plan sets for delivery to our Building Standards staff who will
prepare the Permit and obtain the signature of the Chief Building Official.
Dublin activities from receipt of approvable mylars to call for permit pick-up are
usually completed in 2 to 5 work days. Building Standards staff will contact you
when the permit is ready for pick-up. Please call if you have any questions.

This matter came up for hearing Monday, October 22, 2007 in
Reynoldsburg, Ohio on an appeal from Adjudication Order 2007-02, dated
October 3, 2007, issued by the City of Dublin Building Department.
Said adjudication order involved the premises known as Faith Community
Church.
Based on evidence adduced by, and representations of the
Appellant and the Appellee, the Appellant appealed the adjudication
order.
The Board having determined that inasmuch as it would not be
contrary to the public interest and unnecessary hardship would result
if a literal enforcement of the Ohio Basic Building Code and/or the
Ohio Fire Code was required, a variance against the adjudication order
is given. Adjudication order is described as:
OBC Table 601 - Requires that interior and exterior bearing walls
of construction Type VA be fire-resistance rated one hour.
OBC 704.5 Fire Resistance Ratings. Exterior walls shall be fire
resistance rated in accordance with Tables 601 and 602. The fire
resistance rating of exterior walls with a fire separation distance of
greater than 5 feet (1524 mm) shall be rated for exposure to fire from
the inside. The fire resistance rating of exterior walls with a fire
separation distance of 5 feet (1524 mm) or less shall be rated for
exposure to fire from both sides.
Variance granted noting the no objection from the Building and
Fire Official.
The Board declares that any conditions required as part of the
variance are inseparable and must be complied with in full; variance
is dependent upon compliance with all conditions herein stated and
lack of compliance with any portion of these conditions shall negate
the entire variance. In addition, to any conditions to the variance
shall be incorporated into final Permit Drawings for examination and

any appropriate fees shall be paid.
Any party adversely affected by this order may appeal to the
Court of Common Pleas of the county in which he is a resident or in
which the premises affected by this order is located. In accordance
with Section 119.12, R.C., any party desiring to appeal shall file a
notice of appeal with the Board of Building Appeals and a copy with
the Court, within fifteen (15) days after the mailing of this order,
which notice of appeal shall set forth the order appealed from and the
grounds of appeal.

Further, pursuant to Section 3781.031 of the ORC, any work on the above

a. The construction documents identify Gypsum Association Assembly RC2601
as the required assembly to accomplish the ono-hour fire-resistance rating. GA
RC2601 requires 2layers 5/8" drywall to achieve the rating
b. Only a single layer of drywall has been installed and the layer that is installed
has been penetrated to receive the top plate of the rated corridor walls.
i. Install the assembly as required by the Gypsum Association.
Relief Sought: The church would prefer not to install the extra layer because of financial
burdens and fime delays and because it is not consistent with previous deta+ls regarding
prior construction on this properly approved by the City of Dublin.

the owner and the proposed operating level of food preparation
will not generate snfficient grease laden vapors to require the
installation of a Type I commercial hood. This decision is subject
to review by the Fire Marshal during annual inspections.

compliance with the provisions of this code and other laws that are enforced by the department
of building safety.
a. The Commercial Inspection Sign-Off sheet, provided to the contractor when the permit for
construction was issued, requires a Masonry Wall Grout inspection. The wall has been
constructed without the opportunity for the required inspection by City of Dublin staff.
i Provide a report frnm one of the state of Ohio Approved Testing and Inspection
Agencies, found in Appendiz O of the 2007 OBC, that confzrms the location of the
reinforcing in the wall and certifies the reinforcing and the grout to comply with

OBC 704.5 FIRE-RESISTANCE RATINGS.
Exterior walls shall be fire-resistance rated in accordance with Tables 601 and 602 . The fire-
resistance rating of exterior walls with a fire separation distance of greater than 5 feet (1524 mm)
shall be rated for exposure to fire from the inside. The fire-resistance rating of exterior walls with
a fire separation distance of 5 feet (1524 mm) or less shall be rated for exposure to fire from both
sides.

The drawings through Revision 5 with modifications 6, 7 and 8 and the responses to Correction Request
#7 dated October 3, 2007 resulting from a field inspection on September 19, 2007 are APPROVED This
APPROVAL does not preclude the acquisition of separate permits for plumbing, mechanical, sprinkler or
electric. Permits and fees for these items must be obtained from the Building Division prior to the start of
any work in these areas.
Item 1 109.3.8 Other inspections. In addition to the inspections specified above, the building
official is authorized to make or require other inspections of any construction work to ascertain
compliance with the provisions of this code and other laws that are enforced by the department
of building safety.
a. The Commercial Inspection Sign-Off sheet, provided to the contractor when the permit
for construction was issued, requires a Masonry Wall Grout inspection. The wall has
been constructed without the opportunity for the required inspection by City of Dublin
staff.
i. Provide a report from one of the state of Ohio Approved Testing and Inspection
Agencies, found in Appendix O of the 2007 OBC, that confirms the location of the
reinforcing in the wall and certifies the reinforcing and the grout to comply with
Masonry Notes 4, 4a-c, Sa and Sc on sheet S2.2 of 20 and section B/A5.2 of 20.
Item 2 OBC Table 601 requires that interior and exterior bearing walls be fire-resistance rated one

b. Truss construction documents shall be prepared by a registered design professional and
shall be provided to the building official and approved prior to installation. These
construction documents shall include, at a minimum, the information specified in OBC

i. Reviewer notes that the profile for Truss A1 shows a single truss that would
penetrate the Fire Barrier separating the 2 Fire Areas. Two trusses will be
required each bearing on the wall
c. Please revise the Code Compliance Notes on CSI. l to reflect the new area of the total
area of the existing and new construction. Please also indicate on the Occupancy
Egress Plan the extent of any Fire Areas in the new or proposed construction separated
by Fire Barriers that are in compliance with the requirements of OBC 706
d. Provide a detail explaining the method to be used to transfer the load from the south

e. Please include Door 203 in the Door Schedule and provide information regarding the
type and fire rating of Door 202D
f. Provide a Fire Alarm Pull Station at the exit from the east end of corridor 201 and a

Item 2 Please confirm that the outlet in the east wall of Corridor 200 that is on the other side of the
wall from the relocated food warmer complies with the exception 1 to OBC 712.3.2 or
relocate the corridor wall outlet.
Item 3 OBC 716.2 Installation. Fire dampers, smoke dampers, combination fire/smoke dampers and
ceiling dampers located within air distribution and smoke control systems shall be installed in
accordance with the requirements of this section, the manufacturer's installation instructions
and listing.
a. Provide Fire Dampers in all Ducts penetrating the fire rated corridor walls.
Item 4 OBC 703.3 Alternative methods for determining fire resistance. The application of any of
the alternative methods listed in this section shall be based on the fire exposure and acceptance
criteria specified in ASTM E 119. The required fire resistance of a building element shall be
permitted to be established by any of the following methods or procedures:
1. Fire-resistance designs documented in referenced publications approved in
accordance with this code.
2. Prescriptive designs of fire-resistance-rated building elements as prescribed in Section

a. UL 425 has not been tested for the configuration of the wall Fire Barrier detailed on
SheetAl.1 and Section B/A5.2. Please revise the drawings to comply.
The following items are required for code compliance, but are not necessarily covered in detail in the
construction documents. This list is a reminder to the design professional and contractors of issues, which
are to be satisfactorily dealt with in the field:
Item A. OBC 106.3.1 Approval of construction documents. When the construction documents
have been determined to conform to the applicable provisions of the rules of the Board in
accordance with the procedures set forth in Section 105.3.1, the building official shall
endorse or stamp such plans as approved. One set of construction documents shall be kept
by the building official. The other set(s) shall be returned to the applicant, kept at the
work site and shall be open for inspection by the building official or the building official's
designated representative.
Item B. OBC 105.7.2 Posting. The certificate of plan approval shall be posted in a conspicuous
location outside of the building and in the front part of the premises on which is, or will be
the approved building or equipment to which the approved plans relate. The owner and
the contractor shall, so fax as possible, preserve and keep the certificate posted until the
completion of the work to which the approved plans relate.
Item C. OBC 106.4 Amended construction documents. If substantive changes to the building
are contemplated after first document submission, or during construction, those changes
must be submitted to the building official for review and approval prior to those changes
being executed. The building official may waive this requirement in the instance of an
emergency repair, or similar instance.
Item D. OBC 109.1 General. Construction or work for which an approval is required shall be
subject to inspection by the building official and such construction or work shall remain
accessible and exposed for inspection purposes until approved. This includes penetrations
of fire resistance-rated assemblies, firestopping and draftstopping (Chapter 7 OBC),
mechanical work; piping, ducts and systems (Chapter 3 OMC), structural members and
connections (Chapter 16 OBC), and electrical work (Chapter 27 OBC). All systems and
elements covered by code are to be inspected and approved before being covered.
Item E. OBC 804.3 Testing and identification. Floor covering materials shall be tested by an
approved agency in accordance with NFPA 253 and identified by a hang tag or other
suitable method so as to identify the manufacturer or supplier and style, and shall indicate
the interior floor finish or floor covering classification according to Section 804.2 . Carpet-
type floor coverings shall be tested as proposed for use, including underlayment. Test
reports confirming the information provided in the manufacturer's product identification
shall be furnished to the building official upon request.

Item F. OBC 1103.1 Where required. Buildings and structures, temporary or permanent,
including their associated sites and facilities, shall be accessible to persons with physical
disabilities.
OBC 1103.2.1 Specific requirements. Accessibility is required in buildings and
facilities, or portions thereof, to the extent indicated in ADAAG Section 4.1 and this
chapter.
Item G. All electrical will comply with the requirements of Article 27 OBC and the National
Electrical Code, NFPA 70, OBC approved edition and is subject to the approval of the
electrical field inspector.
Item H. The existing electrical conductors, if removed, abandoned, or altered, shall be accomplished
to the electrical field inspector's approval. Electrical work found not to meet the code's
standards shall be corrected to the electrical field inspector's approval.
This review does not preclude the necessity to conform to provisions which may have been omitted or
overlooked in the review process, but which are requirements of the code. Ultimate responsibility for legal
compliance with the Standards of Safety rests with the registered design professional, the tenant and the owner.
This review is based upon the information contained within the submitted drawings and related documents.
Once corrections are made, submit four (4) complete sets of plans to the City of Dublin, Division of Building
Standards.

the alternative methods listed in this section shall be based on the fire exposure and acceptance
criteria specified in ASTM E 119. The required fire resistance of a building element shall be
permitted to be established by any of the following methods or procedures:
1. Fire-resistance designs documented in referenced publications approved in
accordance with this code.
2. Prescriptive designs of fire-resistance-rated building elements as prescribed in Section
720.
3. Calculations in accordance with Section 721 .
a. UL 425 has not been tested for the configuration of the wall Fire Barrier detailed on
Sheet A1.1 and Section B/A5.2. Please revise the drawings to comply.
The following items are required for code compliance, but are not necessarily covered in detail in the
construction documents. This list is a reminder to the design professional and contractors of issues, which
are to be satisfactorily dealt with in the field:
Item A. OBC 106.3.1 Approval of construction documents. When the construction documents
have been determined to conform to the applicable provisions of the rules of the Board in
accordance with the procedures set forth in Section 105.3.1, the building official shall
endorse or stamp such plans as approved. One set of construction documents shall be kept
by the building official. The other set(s) shall be returned to the applicant, kept at the
work site and shall be open for inspection by the building official or the building official's
designated representative.
Item B. OBC 105.7.2 Posting. The certificate of plan approval shall be posted in a conspicuous
location outside of the building and in the front part of the premises on which is, or will be
the approved building or equipment to which the approved plans relate. The owner and
the contractor shall, so far as possible, preserve and keep the certificate posted until the
completion of the work to which the approved plans relate.
Item C. OBC 106.4 Amended construction documents. If substantive changes to the building
are contemplated after first document submission, or during construction, those changes
must be submitted to the building official for review and approval prior to those changes
being executed. The building official may waive this requirement in the instance of an
emergency repair, or similar instance.
Item D. OBC 109.1 General. Construction or work for which an approval is required shall be
subject to inspection by the building official and such construction or work shall remain
accessible and exposed for inspection purposes until approved. This includes penetrations
offire resistance-rated assemblies, firestopping and draftstopping (Chapter 7 OBC),
mechanical work; piping, ducts and systems (Chapter 3 OMC), structural members and
connections (Chapter 16 OBC), and electrical work (Chapter 27 OBC). All systems and
elements covered by code are to be inspected and approved before being covered.
Item E. OBC 804.3 Testing and identification. Floor covering materials shall be tested by an

approved agency in accordance with NFPA 253 and identified by a hang tag or other
suitable method so as to identify the manufacturer or supplier and style, and shall indicate
the interior floor finish or floor covering classification according to Section 804.2 . Carpet-
type floor coverings shall be tested as proposed for use, including underlayment. Test
reports confirming the information provided in the manufacturer's product identification
shall be furnished to the building official upon request.
Item F. OBC 1103.1 Where required. Buildings and structures, temporary or permanent,
including their associated sites and facilities, shall be accessible to persons with physical
disabilities.
OBC 1103.2.1 Specific requirements. Accessibility is required in buildings and
facilities, or portions thereof, to the extent indicated in ADAAG Section 4.1 and this
chapter.
Item G. All electrical will comply with the requirements of Article 27 OBC and the National
Electrical Code, NFPA 70, OBC approved edition and is subject to the approval of the
electrical field inspector.
Item H. The existing electrical conductors, if removed, abandoned, or altered, shall be accomplished
to the electrical field inspector's approval. Electrical work found not to meet the code's
standards shall be corrected to the electrical field inspector's approval.
This review does not preclude the necessity to conform to provisions which may have been omitted or
overlooked in the review process, but which are requirements of the code. Ultimate responsibility for legal
compliance with the Standards of Safety rests with the registered design professional, the tenant and the owner.
This review is based upon the information contained within the submitted drawings and related documents.
Once corrections are made, submit four (4) complete sets of plans to the City of Dublin, Division of Building
Standards.

This Phased Plan Approval SUPERCEDES ALL PREVIOUS PHASED APPROVALS and is for ALL
EXCEPT FIRE ALARMS, TRUSSES, CORRIDOR DOORS AND DUCTS PENETRATING FIRE
RATED CONSTRUCTION.
Item 1 The submitted documents were incomplete. Further Plan Approvals can not be issued until
the following information is submitted for review and is approved
a. Fire Alarm Drawings indicating modifications to the existing system shall be submitted
for review; contain all information required by NFPA 72-2002 4.5.1.1 and explained in
Annex A4.5.1.1; and be approved before any equipment is installed or remodeled. A
separate Permit from the city of Dublin is required.
b. Truss construction documents shall be prepared by a registered design professional and
shall be provided to the building off cial and approved prior to installation. These
construction documents shall include, at a minimum, the information specified in OBC
2304.4.1
Item 2 OBC 716.2 Installation. Fire dampers, smoke dampers, combination fire/smoke dampers and
ceiling dampers located within air distribution and smoke control systems shall be installed in
accordance with the requirements of this section, the manufacturer's installation instructions
and listing.
a. The resubmission did not result in compliance. Smoke and Fire Dampers are
required in corridor walls by OBC 716.5.4 and 716.5.4.1 with noted exceptions.
Several of the ducts penetrating the corridor walls did not qualify for the exception.
Fire Dampers are required in all penetrations of Fire Walls by 716.5.1
Item 3 715.3.3 Door assemblies in corridors and smoke barriers. ... Fire door assemblies shall
also meet the requirements for a smoke- and draft-control door assembly tested in accordance
with IJL 1784 with an artificial bottom seal installed across the full width of the bottom of the
door assembly. The air leakage rate of the door assembly shall not exceed 3.0 cfin per square
foot (0.01524 m 3/s x m 2) of door opening at 0.10 inch (24.9 Pa) of water for both the
ambient temperature and elevated temperature tests. Louvers shall be prohibited.

of the Ohio Building Code (2005) for the purpose of issuing a Phased Plan Approval only. The revievv was based

The structural plans are sealed by Jeffrey M. Reker, Architect #11521, all to comply with the requirements of the
OBC Section 1063.4.1 and have been reviewed, therefore,'in accord with OBC Section 104.1.
OBC 106.3.3 Phased approval. The building official shall issue an approval for the construction of foundations or
any other part of a building or structure before the construction documents for the «-hole building or structure have
been submitted, provided that adequate information and detailed statements have been filed complying with pertinent
requirements of this code. The holder of such approval for the foundation or other parts of a building or structure
shall proceed at the holder's own risk with the buildincy operation and without assurance that an approval for the
entire structure will be granted. Such approvals shall be issuetl for various stages in the sequence of construction
provided that all information and data required by the code for that portion of the building or structure has been
submitted. The holder of a phasecl plan approval may proceed only to the point for which approval has been given.

Item 1. The square footage for this addition (8,062 Sq. Fz) includes the alternate Bid V.
Item 2. Design loads Note #1 on Sheet S2.2 should state "soil bearing values to be confirmed
by the Architect of Record"; since he has the ultimate responsibility.
Item 3. It is not expected that the future kitchen range wil[ be used for large scale cooking tliat
produces "grease laden vapors "; see OMC 507.
Item 4. Provide separate drawings for the fire alarm system; see OBC 907.2.3.
Item 5. Provide a door schedule with fire ratings and hard?vare, including closers, oi: all doors
in the rated corridor walls.
Item 6. Architect Jeff Reker's letter of November 22, 2006, was received January 25, 2007 and
is a part of this Phased Plan Approval.
The following items are required for code compliance, but are not necessarily covered in detail in the construction
documents. This list is a reminder to the design professional and contrnctors of issues, whiclr are to be
satisfactorily dealt with in the field:
Item 1. OBC 109.2 Lot line markers required. Before any work is started in the construction of a
building or an addition to a building to which the rules of the Board are applicable under Section
101.2, all boundary lines shall be clearly marked at their intersections with permanent markers or
with markers which are offset at a distance which is of record with the owner.
Item 2. OBC 106.31 Approval of eonstruction doeuments. When the construction documents have
been determined to conform to the applicable provisions of the rules of the Board in accordance
with the procedures set forth in Section 1053.1, the building official shall endorse or stamp such
plans as approved. One set of construction documents shall be kept by the building official. The
other set(s) shall be returned to the applicant, kept at the work site and shall be open for inspection
by the building official or the building official's designated representative.
Item 3. OBC 105.7.2 Posting. The certificate of plan approval shall be posted in a conspicuous location
outside of the building and in the front part of the premises on which is, or «ill be the approved
building or equipment to which the approved plans relate. The o«ner and the contractor shall, so
far as possible, preserve and keep the certificate posted until the completion of the %vork to which
the approved plans relate.
Item 4. OBC 106.4 Amended construction documents. If substantive changes to the buildina are
contemplated after first document submission, or during construcrion, those changes must be
submitted to the building official for review and approval prior to those changes beina executed.
The building official may waive this requirement in the instance of an emeraency repair, or similar
instance.
Item 5. OBC 109.1 General. Construction or work for which an approval is required shall be subject to
inspection by the building official and such construction or work shall remain accessible and
exposed for inspection purposes until approved.

This includes firestopping and draftstopping (Chapter 7 OBC), mechanical lvork; piping, ducts
and systems (Chapter 3 OMC), structural members and connections (Chapter 16 OBC), and
electrical work (Chapter 27 OBC). All systems and elements covered by code are to be inspected
and approved before being covered.
Item 6. OBC 1103.1 Where required. Buildings and structures, temporary or permanent, including their
associated sites and facilities, shall be accessible to persons with physical disabilities.
OBC 1103.2.1 Specific requirements. Accessibility is required in buildings and facilities, or
portions thereof, to the extent indicated in ADAAG Section 4.1 and this chapter.
Item 7. All electrical will comply with the requirements of Article 27 OBC and the \ational Electrical
Code, NfiPA 70, OBC approved edition and is subject to the approval of the electrical field
inspector.
Item 8. The existing electrical conductors, if removed, abandoned, or altered, shall be accomplished to the
electrical field inspector's approval. Electrical work found not to meet the code's standards shall
be corrected to the electrical field inspector's approval.
The attached letter from Washington Township Fire Department shall serve as a reference to the "Phased Approval"
letter from Martin Strayer, City of Dublin Plans Examiner. Unless otherwise noted, the "Phased Approval" letter
and the referenced letter shall serve as an approval from Jeffrey S. Tyler, Chief Building OfficiaUDirector of
Building Standards.
The start of construction indicates acceptance of all of the conditions listed above. This review does not preclude the
necessity to conform with provisions which may have been omitted or overlooked in the review process, but which
are requirements of the code. Ultimate responsibility for legal compliance with the Standards of Safety rest with the
registered design professional, the tenant and the owner. Phased Plan Approval will be valid only upon receipt by
the City of Dublin, Division of Building Standards, of one signed copy of this Phased Plan Approval to the submitted
plans.

to the Washington Township Fire Department regarding rules of the 2005 Ohio Building Code (OBC) and the 2002 Dublin
Fire Code (DFC) relating to fire prevention.
We are able to recommend approval of the plans as submitted. We note only the following:
This review is silent on the matter of future expansion. Future expansion will have to be considered on the specifics
of the nature of the expansion and the requirements of the codes then current.

• The water service for these buildings will need to be reviewed by the City of Columbus Water Department. Incorporate

Landscape and Tree Preservation Plan Requirements
Tree Preservation
• All tree survey information must be shown at a scale no less than 1-inch equal to 50-feet. (The
tree preservation plan can be combined with the landscape plan provided a scale 1-inch equals
20-feet is used and that there is a visible difference in the symbol(s) used for existing plant
material versus proposed plant material.) All existing trees of 6-inches caliper or larger located
on site and within 30-feet of perimeter of site, existing utilities, streams, ditches, wetlands,
ponds, existing topography, existing structures or features.
• Identify the size, species, condition and critical root zone of all trees shown on the tree survey.
Note which trees are to be removed if applicable.
• Tree protection detail and tree preservation/protection notes need to be included in plans.
• Tree protection fencing is to be shown on all relevant sheets (grading site, etc.) in addition to
being shown on the tree preservation plan. Reference should be made to see tree preservation
plan for details.
• Tree preservation area/building activity areas are to be shown including ingress/egress, parking
areas, and storage areas.
• Locations for clearing, grubbing, grading, and excavation are to be shown.
• All tree survey information is to have the stamp and signature of the design
engineer/surveyor/landscape architect/architect licensed in the State of Ohio.
Landscape Plan Content
In addition to the tree preservation outlined above, the following information will be required for a
Landscape Plan for the project:
• Landscape plan must have the stamp and signature of a landscape architect licensed in the State
of Ohio.
• Plans are to be at a readable scale of 1-inch equal to 20-feet.
• Show all proposed and existing contours on the landscape plan.
• Proposed walkway to the city sidewalk cuts into toe of existing mound; show how grading will
be completed.
• Show light poles.
• Show all buildings within 30-feet of site perimeter; label and dimension.
• Provide a plant list that identifies the proposed plant material by botanical name, common
name, installation size, center planting dimensions and quantity.
• Permanent seed mix is not identified.

• Per code, there shall be a minimum of one tree for every 5,000 square feet of ground coverage
with a minimum installation size of 2-inch caliper. There should be 2 additional trees at 2-inch
caliper based upon square footage of addition.
• Show any existing and proposed foundation plantings.
• There shall be tree plantings equal to one-inch in tree size for every 1,500 square feet of
building ground coverage.
• Property Perimeter Requirements - Single-family residential exists to the north and property
perimeter landscape requires one tree per 40-feet of lineal boundary plus a continuous 6-foot
high planting, hedge, fence, wall or earth mound.
• Vehicular Use Area Perimeter Requirements - The vehicular use area that fronts Woerner-
Temple Road meets screening requirements. However, the vehicular use area that is closest to
Wilcox Road will need to be brought into compliance with the screening code.
• ScreeninQ for Service Structures - All service structures and mechanical units are to be
screened per code.
• Street Tree Requirements - Show all street trees installed along Woerner Temple Road and
Wilcox Road.

coordinated with any applicable comments by the Division of Building Standards, Engineering and the

We have reviewed the plans for the referenced project dated May 29, 2006 for matters of concem to the Washington
Township Fire Department regazding rules of the 2005 Ohio Building Code (OBC) and the 2002 Dublin Fire Code (DFC)
relating to fire prevention.
We are unable to recommend approval of the plans as submitted.
DFC 503.1 Fire Apparatus Access Roads (FAAR), Where Required, DFC 503.1.1 Buildings and facilities...
The fire apparatus access road shall ... extend to within 150 fcet (45 720 mm) of all portions of the facility or any
portion of the exterior wall of the first story of the building as measured by an approved route around the exterior of
the building or facility.
a. The existing drive does not extend to within 1 SO jeet of all portions of the proposed facility. Until an FAAR
that conforms to the requirements of the DFC rs provided the increased area permitted by OBC 506 can not
be taken.
b. "Future Facpansion " is noted on the Site Plan. As indicated the expansion could not comply with the
requirements of the DFC.
DFC 503.2 Specifications.
DFC 503.2.2 Authority. The code official shall have the authority to require an increase in the minimum acc,ess
widths where they are inadequate for fire or rescue operations.
DFC 503.2.3 Surface: Fire apparatus access roads shall be designed and maintained to support the imposed loads of
fire apparatus (75,000 pounds) and shall be surfaced so as to provide all-weather driving capabilities.
DFC 503.2.4 Turning Radius. The turning radius on any fire apparatus access road, public or private, that provides
access to a structure or equipment, shall be a minimum turning radius of 45 feet on the outside and 25 feet on the
inside.
DFC 503.2.5 Dead ends. Dead-end fire apparatus access roads in excess of 150 feet (45 720 mm) in length shall be
provided with an approved area for turning azound fire apparatus.
c. The existing drive is a dead end is 700+ f¢¢t and is required by DFCAppendix D: Figure DI03.1 to have a
100 foot diameter Cul-de-sac at its termination. A dead end FAAR is required to be 28' wide.

wide (7925 mm) to 32 feet wide (9754 mm) shall be posted on one side of the road as a fire lane. FAAR's between
26 and 32 feet wide and having fire hydrants along one side shall be posted on the hydrant side of the street; FAAR's
consisting of a cul-de-sac shall be posted around the entire perimeter of the cul-de-sac, unless that radius of the cul-
de-sac is 65 feet or greater; if an island is located in the center of the cul-de-sac, it shall also be posted.
a. Coordinate required signage with the Fire Marshal.
DFC 504.1 Required Access. Exterior doors and openings required by this code or the OBC shall be maintained
readily accessible for emergency access by the fire department. An approved access walkway leading from the fire
apparatus access roads to exterior openings shall be provided when required by the code official.
a. Access to tke exits on the north and east walls of the structure have been made inaccessible by the storm
detention area and the condenser enclosure walls. Provide paved access to the exits from the FAAR.
OBC 1023.6 Access to a public way. The exit discharge shall provide a direct and unobstructed access to a public
way. Exception: Where access to a public way cannot be provided, a safe dispersal area shall be provided where all
of the following are met:
1. The area shall be of a size to accommodate at least 5 square feet (0.28 m2) for each person.
2. T'he area shall be located on the same property at least 50 feet (15 240 mm) away from the building
requiring egress.
3. The area shall be permanendy maintained and identified as a safe dispersal area.
4. The area shall be provided with a safe and unobstructed path of travel from the building.
a. Please construct sidewalks that provide direct and unobstructed access to a public way or a dispersal area
complying with the requirements oJexceptions 1 thru 4. All required exits must leacl to the public way.
5. DFC 508.5.1 Where Required. Where a portion of the facility or building hereafter constructed or moved into or
within the jurisdicdon is more than 400 feet (122 m) from a hydrant on a fire apparatus access road, as measured by
an approved route around the exterior of the facility or building, on-site fire hydrants shall be provided and
maintained where required by the fire official. Exception 2 permits the distance requirement to be increased to 600
feet if the structure is equipped throughout with an approved automadc sprinkler system.
a. Please locate the nearest fire hydrants on the site plan
6. Please request confirmation from the architect of the construction type of the existing church construction.
Please request that details be provided indicating how the one hour rated construction will be provided/maintained
for the existing north bearing wall when the exterior masonry is removed.
OMC 606.2.2 Commoa supply and return air systems. Where multiple air-handling systems share common
supply or return air ducts or plenums with a combined design capacity greater than 2,000 cfin (0.9 m3/s), the retum
air system shall be provided with smoke detectors in accordance with Section 606.2.1.

We have reviewed the plan submission made by your office on September 13, 2006 for this proj ect. Unforlunately, the civil set
of drawings and proof of compliance with the City's stormwater management ordinance required for projects like this was not
submitted. Please find attached a copy of the page by requirements for the civil set of plans. Keep in mind this list was
established for new builds, you should be able to reduce the amount of some of the requirements since this only and addition to
the building. Once the proper site plans are submitted a full review will be completed. In addition, I offer the following general
comments.
General Comments
• The water service for these buildings will need to be reviewed by the City of Columbus Water Department. Incorporate
any comments from them into the plans.
• The Site Improvement Plans will be required to be submitted on 22 by 34-inch 4-mil mylars and in electronic format
compatible with AutoCAD once final approval is given.
• General Notes are to be provided as given in the Division of Engineering Administrative Policy 95-01 l. This
information is available electronically on our website www.dublin.oh.us. Some updates need to be made on these plans.
• The stamp and signature of the design professional that prepared this drawing will be needed on the final Mylar.
• Provide additional spot grades for the handicap spaces to ensure that no grades are greater than 2% in any direction.
• Please provide the signature and stamp of the surveyor that performed the survey.
• Additional calculations (critical storm calculations, inflow/outflow hydrographs, routing calculations, etc.)
demonstrating compliance with the Stormwater Regulations are to be submitted and in the format described within the
Division of Engineering Draft Guidance Document available on our website www.dublin.oh.us.
• Add the following note above the City of Dublin signatures: Signatures below signify only concurrence with the general
purposes and general location of the project. All technical details remain the responsibility of the Engineer preparing
the plans.
• Add a note to the proposed curb cut that this will be installed per City of Dublin Standard Drawing 603.1-96 and
requires inspection. Please review the grading to make sure the, required grades within the approach are incorporated.
• Show the intersection visibility triangle for the driveway in accordance to the Division of Engineering Administrative
Policy 95-013. Use a stop sign controlled intersection.
• The last run of pipe to the public system is required to be reinforced concrete pipe. Please revise accordingly.
• OEPA is now requiring that a PTI be issued before any construction of the sanitary sewer system can be completed.
The plans may also need to be reviewed and signed by the City of Columbus. Contacts with these entities should be
made concurrently with the City of Dublin review.

being returned to your attention for revision. For your convenience, this letter is to summarize any and all revisions required for
zoning compliance only. Please review the contents of this letter with all comments "redlined" on the set marked "Z."
Please return all redlined plans with vour resubmittal to expedite the next round of permit plan review.
The permit sets submitted for review do not include detailed civil engineer site construction drawings, landscape plans or tree
preservation plans as required by the City of Dublin. T'he civil, landscape and tree preservation plans must contain specific
information in order for the staff to determine that the proposed project will be in compliance with all applicable codes,
regulations and requirements. Please refer to the attached site plan guidelines for the requirements of the civil plans.
A specific and detailed list of all plan review requirements would be lengthy and difficult to convey in this form. The staff
would like to suggest a meeting with you and your design team to review the content and format of any revised plans
before any plans are resubmitted for review. In that way, specific and detailed information concerning engineering,
landscape and zoning requirements can be discussed. Please contact Steve Snyder of the City of Dublin Review Services staff
