Foundation Observation
Dear Michelle:
At your request, we vlsited the site on November 12, 2007 to observe the concrete foundation walls. The
foHowing is s summary of our site visit and recommendations for repairs.
The work performed was limfted to visual observa6on of only those elements which are unconcealed by
finishes. No measurements, testiny or calculations were performed to evaluate the overail structurai integriry
of the building. Statementa containsd within th4a letter are based on observations made at the time of the
investigation. All opfniona are baaed on sound engineering judgment. Shirk & O'Donovan Consulting
Engineers, Incorporated cannot and does not waRant pre-existing or future dwelling conditions which may
conflict with building code reguiations.
SlTE OBSERVATIONS
The convent(onai wood-framed multi-family residence is supported with 8-inch thick by 9-foot tall concrete
foundation waHs. We observed slx cracks at the time of our inspection.
Front Foundation Wall-Two full helght veRicai cracks are present in thls wall. Each one is near the midpoint
of the two sections comprisinq the front wall. These cracks have a maximum wldth of 0.060 inches.
Right Foundation Wall -- Two full height vertical cracks are present in this walf. One verticai crack is present
propagating from the lower lett corner of the rlght front beam pocket and has a maximum width of 3116-inch.
The other crack is bcated approximately midpaint between the rear steel beam and the rear foundation wall.
This crack has a maximum width of 0.040 inches.
Rear Foundation WaH - Two full height vertical cracks are present in this wall. One crack propagates from the
lower left corner of the left crawl access opening and has a maximum width of 0.050 inches. The other crack
is near the midpoint of the left section of wall with a maximum wfdth of 0.040 inches.
Left Foundation Wall - We observed no significant cracks in this waN.
We observed no significant displacement in the inter(or face of the wafl across any of the cracks.

RECOMMENDATIONS
All of the defects obsenred are characteristic of concrete shrinkage. Concrete shrinks as it cures and the
degree of shrinkage varies. There are many variables that affect the shrinkage of the concrete. 7hese
include, but are not limited to, temperature and humidity after the pour, design mix, presence of admixtures or
water added at the jobsite. Shrinkage cracks ere very common and are normally minor in nature. This rype of
crack normally forms at window/door opening corners and at the form-tie loca6ons. 7hese discontinuities
create a"weak" location within the wall and provide a(ocation for the crack to form. This is similar to control
joints in concrete slabs where the intent is to force the shrinkage cracks to occur at controfled locations.
Shrinkage cracks do not signiflcantly impact the structural performance of the concrete foundation walls, but
do provide a potential means of moisture penetration and aesthetic concems.
The crack observed at the right front beam pocket is unusually large and warrants repair. This crack shouid
be repaired by epoxy injection with Simpson ETI Injection Epoxy (or equal) per the manufacturer's
recommendations. All other cracks observed are minor shrinkage cracks and are not structurally significant
but warrant repair to prevent potential moisture penetratian. 7hese cracks should be sealed with Simpson
Crack-Pac F1ex-1-120 Polyurethane Crack Sealer (or equal) or Pl. Polyurethane ID Concrete & Masonry Sealant
per the manufacturer's reoommendations. The above repairs will seal the cracks from moisture penetration
and restore the foundation walls to their original design intent.

Dear Michelle:
At your request, we have reviewed several walls that have been extended in height. Presently, these 2x4
waifs are constructed in two secfions. The lower section of the wall is framed to 9'-1 1/8° with a double top
plate and the upper section extends 1'-3" above. Based upon digital photos, provided by you, the vertical
studs align between the two sections of wall. The wa{I sections are sheathed with either wood panels or rigid
insufation. This method of construction creates a hinge point !n the wail, compromising its stability.
Modify the walls by connecting the bwer and upper studs with a Simpson LSTA18 strap tie each face of each
stud. Center the strap verticatly on the trRpte plate in the wall. lt is acceptable to install the straps over wood
sheathing, but any foam sheathing (blue board) must be removed and the strap attached directly to the studs.
Where wood sheathing is on the upper portion and toam sheathing is on the lower portion, remove the foam
and replace with OSB furring for the length of the strap. Match the number of king studs at openings in the
lower portion of the wall in the upper section and strap each stud as indicated above. In addition, attach the
bottom plate of the upper section to the double top plate af the (ower section with 16d r,sils spar2d at 8 inches
on-center. '

OBC 106.3.1 Approval of construction documents. When the construction documents have been
determined to conform to the applicable provisions of the rules of the Board in accordance with the
procedures set forth in Section 105.3.1, the building official shall endorse or stamp such plans as
approved. One set of construction documents shall be kept by the building official. The other set(s)
shall be returned to the applicant, kept at the work site and shall be open for inspection by the
building official or the building official's designated representative.
This APPROVAL does not preclude the acquisition of separate permits for plumbing, mechanical,

Item 1. The roof trusses are designed and built by Stark Truss in Washington Court House,
Ohio. The floor trusses are supplied by North Santiam Lumber and are LP (Louisiana
Pacific) design, out of the book.
The following items are required for code compliance, but are not necessarily covered in detail in the construction
documents. This list is a remincler to the design professional and contractors of issues which are to be satisfactorily
dealt with in the field.
Item 1. OBC 109.2 Lot line markers required. Before any work is started in the construction of a
building or an addition to a building to which the rules of the Board are applicable under Section
101.2, all boundary lines shall be clearly marked at their intersections with permanent markers or
with markers which are offset at a distance which is of record with the owner.
Item 2. OBC 106.3.1 Approval of construction documents. When the construction documents have
been deternuned to conform to the applicable provisions of the rules of the Board in accordance
with the procedures set forth in Section 1053.1, the building official shall endorse or stamp such
plans as approved. One set of construction documents shall be kept by the building official. The
other set(s) shall be returned to the applicant, kept at the work site and shall be open for inspection
by the building official or the building official's designated representative.
Item 3. OBC 105.7.2 Posting. The certificate of plan approval shall be posted in a conspicuous location
outside of the building and in the front part of the premises on which is, or will be the approved
building or equipment to which the approved plans relate. The owner and the contractor shall, so
far as possible, preserve and keep the certificate posted until the completion of the work to which
the approved plans relate.
Item 4. OBC 106.4 Amended construction documents. If substantive changes to the building are
contemplated after first document submission, or during construction, those changes must be
submitted to the building official for review and approval prior to those changes being executed.
The building official may waive this requirement in the instance of an emergency repair, or similar
instance.
Item 5. OBC 109.1 GeneraL Construction or wark for which an approval is required shall be subject to
inspection by the building official and such construction or work shall remain accessible and
exposed for inspection puiposes until approved.
This inclucles firestopping and draftstopping (Chapter 7 OBC), mechanical work; piping, ducts
and systems (Chapter 3 OMC), structural members and connections (Chapter 16 OBC), and
electrical work (Chapter 27 OBC). All systems and elements covered by code are to be inspected
and approvecl before being covered.
Item 6. OBC 1103.1 Where required. Buildings and structures, temporary or permanent, inciuding their
associated sites and facilities, shall be accessible to persons with physical disabilities.
OBC 1103.2.1 Specific requirements. Accessibility is required in buildings and facilities, or
portions thereof, to the extent indicated in ADAAG Section 4.1 and this chapter.
Item 7. All electrical will comply with the requirements of Article 27 OBC and the National Electrical
Code, NfiPA 70, OBC approved edition and is subject to the approval of the electrical field
inspector.

Item 8. The existing electrical conductors, if removed, abandoned, or altered, shall be accomplished to the
electrical field inspector's approval. Electrical work found not to meet the code's standards shall
be corrected to the electrical field inspector's approval.
The start of construction indicates acceptance of all of the conditions listed above. This review does not preclude
the necessity to conform with provisions which may have been omitted or overlooked in the review process, but
which are requirements of the code. Ultimate responsibility for legal compliance with the Standards of Safety rest
with the registered design professional, the tenant and the owner. Approval will be valid only upon receipt by the

OBC 106.3.3 Phased approval. The building official shall issue an approval for the construction of foundations or
any other part of a building or structure before the construction documents for the whole building or structure have
been submitted, provided that adequate information and detailed statements have been filed complying with pertinent
requirements of this code. The holder of such approval for the foundarion or other parts of a building or siructure
shall proceed at the holder's own risk with the building operation and without assurance that an approval for the
entire stxucture wili be granted. Such approvals shall be issued for various stages in the sequence of construction
provided that all information and data required by the code for that portion of the building or structure has been
submitted. The holder of a phasedplan approval may proceed only to the point for which approval has been given.

Item 1. Work may not proceed beyond the foundation. A set of floor and roof trusses must be
approved by the architect, developer and supplier. This approved set is to be submitted
for full review by the Ciry of Dublin at 5800 Shier Rings Road.
Item 2. The governing code is OBC 2005 and Chapters 2-10 of the Residential Code of Ohio.
Item 3. The note on SheetAS.Ol calls for "complete fireblocking at unit separation walls":
This is to be field coordinated by the City of Dublin building inspector and the framing
contractor.
Item 4. The drafting symbol (B/f15.01) indicates a rigid insulation board, which is exposed to
the crawl space. The material used is to meet R314.1.1 requirements or be a non-
combustible material as indicated on the outside walL
Item 5. R309.2 Separation required. The garage shall be separated from the residence and
its attic area by not less than'/2-inch (12.7 mm) gypsum board applied to the garage
side. Garages beneath habitable rooms shall be separated from all habitable rooms
above by not less than 5/8-inch (15.9 mm) Type X gypsum board or equivalent.
Where the separation is a floor-ceiling assembly, the structure supporting the
separation shall also be protected by not less than'/Z-inch (12.7 mm) gypsum board
or equivalent.
Item 6. R317.3 Rated penetrations. Penetrations of wall or floor/ceiling assemblies required
to be fire-resistance-rated in accordance with Section R317.1 or R317.2 shall be
protected in accordance with this section.
Item 7. R807.1 Attic access. In buildings with combustible ceiling or roof construction, an
attic access opening shall be provided to attic areas that egceed 30 square feet (2.82)
and have a vertical height of 30 inches (762 mm) or greater.
The rough-framed openings shall not be less than 22 inches by 30 inches (559 mm by
762 mm) and shall be located in a hallway or other readily accessible location. A 30-
inch (762 mm) minimum unobstructed headroom in the attic space shall be provided
at some point above the access opening. See Section M1305.1.3 for access
requirements where mechanical equipment is located in attics.
Item 8. R311.2.2 Under stair protection. Enclosed accessible space under stairs shall have
walls, under stair surface and any soffits protected on the enclosed side with'/Z-inch
(12.7 mm) gypsum board.
