The Revised Fire Alarm/Detection Drawings dated 8-6-07 submitted with the Fire Protection Permit
Application, for the captioned project, has been reviewed for compliance with the provisions of the 20070hio
Building Code (OBC). The review was based upon the following criteria:

contractor is REI Telcom, Certification Number 53-25-1542 he installer has been identified as Jeff Rieser
Certification Numbers 54-25-2380o comply with the requirements of OBC Section 105.3 and have been reviewed,
therefore, in accord with OBC section 105.3.1.4.
This PLAN APPROVAL for the fire protective signaling system only, is issued by the City of Dublin, Division
of Building Standards, in accordance with Section 106.3.1 OBC.
For the information of the Applicant the following item(s) are required for code compliance, but are not
necessarily covered in detail in the construction documents This list isa reminder to the design professional
and contractors of issues which are to be satisfactorily dealt with in the field:
1. The certificate of approval of fire protection plans shall be posted in a conspicuous location outside of the
building or in the front part of the premises on which is, or will be, located the approved building or
equipment. The owner and the contractor shall, so far as possible, preserve and keep said certificate until
the completion of the work to which the approved construction documents relate; Section 105.7.2 OBC.
2. The proposed construction shall conform to the requirements of Chapter 11 OBC and the "Americans with
Disabilities Act Accessibility Guidelines" (ADAAG).

3. Fire Alarm System cable and wiring located in noncombustible plenums shall meet with the requirements
of Section 907 OBC and Article 760, NFPA 70.
4. Provide a record of completion for each completed fire alarm system in accordance with Section 907.17
and NFPA 72 Completion Documents.
5. Upon completion of the fire protective signaling system, all alarm-initiating devices and circuits, alarm-
indicating appliances and circuits, supervisory-signal initiating devices and circuits, signaling line circuits,
and primary and secondary power supplies shall be subjected to a 100 percent acceptance test in
accordance with NFPA 72A and 72E listed in Chapter 35; Section 907.16 OBC.
6. OBC 907 Acceptance testing: When the installation of the detectors is complete, each detector and all
interconnecting wiring for multiple-station detectors shall be subject to a 100-percent acceptance test in
accordance with the household fire warning equipment provisions of NFPA 72 listed in Chapter 35.
7. No electrical work (conduits, cables or other electrical apparatus) may be covered or concealed without
visual inspection and approval by the electrical field inspector as per OBC 109.
8. The design and construction of alterations to the existing electrical systems shall comply with Chapter 27
OBC and NFPA 70.
9. All electrical work will comply with the requirements of Article 27 OBC and the National Electric Code
NFPA 70, OBC approved edition and is subject to the approval of the electrical inspector, in accordance
with Section 109 of the OBC.

Scope of Work for COLUMBUS CARDIOLOGY
This project involves the installation of Siemens fire initiating devices, notification
appliances, and notification panels.
Scope of work includes adding the notification devices to the existing Pad 3
extender panel.
All notification appliances will be synchronized to meet the requirements of the
ADA regulations.
The fire alarm notification panels shall obtain their primary power from 120 VAC,
60 Hz supply provided with a dedicated and secured disconnect switch. A
secondary emergency power supply with capacity for operating the system in
standby mode for 24 hours followed by alarm mode for 15 minutes is included.
All project wiring shall be installed in strict compliance with local codes and the
provisions of NEC Article 760.
The fire alarm system is to be tested in accordance with the NFPA-72. The test
is to be completed and witnessed by the owner's representative.

ADAAG 4.28.1. Alarm systems required to be accessible shall comply with Section 4.28 ADAAG.. At a minimum,
visual signal appliances shall be provided in restrooms and any other general usage areas (e.g_, meeting rooms,
auditoriums, corridors, lobbies and any other area for common use.
Provide visual notification appliances in the following "common use" areas:
1. Each of the Examination Rooms.

By definition, these areas constitute common use and shall be protected in accordance with the referenced code
sections. Furthermore, audible alarms shall be tested in the field for compliance with Section 907 9.2 OBC
pursuant to these areas of common use.
2. OBC 907.9 Alarm notification appliances. Alarm notification appliances shall be provided and shall be listed for
their purpose. Audible and visual alarms shall be installed in accordance with the "ADAAG" and NFPA 72.
ADAAG 4.28.3. Visual alarm initiating signal appliances shall be mounted in accordance with Section 4.28.3
ADAAG.
(6) The appliance shall be placed 80 in. above the highest floor level within the space or 6 in. below the ceiling,
whichever is lower.
In accordance with the provisions of these code sections, the measurement for the appliance ' shall be taken to the
area ' of the visual alarm initiating device lens. This appliance measurement shall placed such that the bottom of
the lens is not less than 80 inches above the f finish floor surface or 6 inches below the ceiling, whichever is lower.
These drawings do not coordinate these requirements. Please verify.
3. OBC 9073.2 Manual fire alarm boxes: The height of the manual fire alarm boxes shall be a minimum of 42 inches
and a maximum of 48 inches measured vertically, form the floor level to the activating handle or lever of the box.
Information demonstrating compliance with this code section was not received with this application.
Please submit for further review.
This review does not preclude the necessity to conform with provisions which may have been omitted or overlooked in the
review process, but which are requirements of the code. Ultimate responsibility for legal compliance with the Standards of
Safety rest with the registered design professional, the tenant and the owner. Plan Approval will be valid only upon receipt
by the City of Dublin, Division of Building Standards, of one signed copy of this addendum.

3. OBC 907.3.2 Manual fire alarm boxes: The height of the manual fire alarm boxes shall be a minimum of 42 inches
and a maximum of 48 inches measured vertically, form the floor level to the activating handle or lever of the box.
