Pizzino, Engineer APPLICANT NO.: 05-200859 The plans dated July 6, 2006, for the captioned project have been reviewed for compliance with the provisions of the Ohio Building Code (2005) for the purpose of issuing a Phased Plan Approval only. The review was based upon the following criteria: Use Group: B Occupancy: 42 Area: 4,192 Sq. Ft. Construction Type: VB Notes: SUBMIT ELECTRICAL, PLUMBING AND HVAC DRAWINGS FOR REVIEW The structural plans are sealed by William J. Pizzino, Engineer #55478, Gary P. Wallace, Engineer #48764 and Xuegang Liu, Engineer #62539, all to comply with the requirements of the OBC Section 106.3.4.1 and have been reviewed, therefore, in accord with OBC Section 104.1. OBC 10633 Phased approval. The building official shall issue an approval for the construction of foundations or any other part of a building or structure before the construction documents for the whole building or structure have been submitted, provided that adequate information and detailed statements have been filed complying with pertinent requirements of this code. The holder of such approval for the foundation or other parts of a building or structure shall proceed at the holder's own risk with the building operation and without assurance that an approval for the entire structure will be granted. Such approvals shall be issued for various stages in the sequence of construction provided that all information and data required by the code for that portion of the building or structure has been submitted. Submit construction documents for electrical, plumbing and HVAC, which will be attached to the July 6, 2006 set of drawings. Item 2. For treatment rooms, add a note to utilize "hospital grade wiring" and emergency lighting per NEC, Article 517. Item 3. Both sides of the draftstopping (note 15, Sheet A-3) need to be accessible. Add another access panel in the ceiling on the south side or add an access panel in the draftstopping; see OBC 717.4. Item 4. During the June 28, 2006 meeting, agreement was reached regarding the soil under the retaining wall in the pond area, which would be evaluated by a soils engineer, Engineer Fizzino and a City of Dublin building inspector. Footing depth, width and the angle of repose will be decided based on this evaluation, after the pond is pumped dry and any muck is removed, this includes both the building footing and the retaining wall on the south side. The following items are required for code compliance, but are not necessarily covered in detail in the construction documents. This list is a reminder to the design professional and contractors of issues, which are to be satisfactorily dealt with in the field: Item 1. OBC 109.2 Lot line markers required. Before any work is started in the construction of a building or an addition to a building to which the rules of the Board are applicable under Section 10 1.2, all boundary lines shall be clearly marked at their intersections with permanent markers or with markers which are offset at a distance which is of record with the owner. Item 2. OBC 1063.1 Approval of construction documents. When the construction documents have been determined to conform to the applicable provisions of the rules of the Board in accordance with the procedures set forth in Section 105.3.1, the building official shall endorse or stamp such plans as approved. One set of construction documents shall be kept by the building official. The other set(s) shall be returned to the applicant, kept at the work site and shall be open for inspection by the building official or the building official's designated representative. Item 3. OBC 105.7.2 Posting. The certificate of plan approval shall be posted in a conspicuous location outside of the building and in the front part of the premises on which is, or will be the approved building or equipment to which the approved plans relate. The owner and the contractor shall, so far as possible, preserve and keep the certificate posted until the completion of the work to which the approved plans relate. Item 4. OBC 106.4 Amended construction documents. If substantive changes to the building are contemplated after first document submission, or during construction, those changes must be submitted to the building official for review and approval prior to those changes being executed. The building official may waive this requirement in the instance of an emergency repair, or similar instance. Item 5. OBC 109.1 General. Construction or work for which an approval is required shall be subject to inspection by the building official and such construction or work shall remain accessible and exposed for inspection purposes until approved. Liquid combustion by-products of condensing appliances shall be collected and discharged to an approved plumbing fixture or disposal area in accordance with the manufacturer's installation instructions. Condensate piping shall be of approved corrosion-resistant material and shall not be smaller than the drain connection on the appliance. Such piping shall maintain a minimum horizontal slope in the direction of discharge of not less than one-eighth unit vertical in 12 units horizontal (1-percent slope). 307.2 Evaporators and cooling coils. Condensate drain systems shall be provided for equipment and appliances containing evaporators or cooling coils. Condensate drain systems shall be designed, constructed and installed in accordance with Sections 307.2.1 through 307.2.4 . 307.2.1 Condensate disposal. Condensate from all cooling coils and evaporators shall be conveyed from the drain pan outlet to an approved place of disposal. Condensate shall not discharge into a street, alley or other areas so as to cause a nuisance. 307.2.2 Drain pipe materials and sizes. Components of the condensate disposal system shall be cast iron, galvanized steel, copper, cross-linked polyethylene, polybutylene, polyethylene, ABS, CPVC or PVC pipe or tubing. All components shall be selected for the pressure and temperature rating of the installation. Condensate waste and drain line size shall be not less than % -inch (19 mm) internal diameter and shall not decrease in size from the drain pan connection to the place of condensate disposal. Where the drain pipes from more than one unit are manifolded together for condensate drainage, the pipe or tubing shall be sized in accordance with an approved method. All horizontal sections of drain piping shall be installed in uniform alignment at a uniform slope. 307.2.3 Auxiliary and secondary drain systems. In addition to the requirements of Section 307.2.1 , a secondary drain or auxiliary drain pan shall be required for each cooling or evaporator coil where damage to any building components will occur as a result of overflow from the equipment drain pan or stoppage in the condensate drain piping. One of the following methods shall be used: 1. An auxiliary drain pan with a separate drain shall be provided under the coils on which condensation will occur. The auxiliary pan drain shall discharge to a conspicuous point of disposal to alert occupants in the event of a stoppage of the primary drain. The pan shall have a minimum depth of 1.5 inches (38 mm), shall not be less than 3 inches (76 mm) larger than the unit or the coil dimensions in width and length and shall be constructed of corrosion-resistant material. Metallic pans shall have a minimum thickness of not less than 0.0276-inch (0.7 mm) galvanized sheet metal. Nonmetallic pans shall have a minimum thickness of not less than 0.0625 inch (1.6 mm). 2. A separate overflow drain line shall be connected to the drain pan provided with the equipment. Such overflow drain shall discharge to a conspicuous point of disposal to alert occupants in the event of a stoppage of the primary drain. The overflow drain line shall connect to the drain pan at a higher level than the primary drain connection. 3. An auxiliary drain pan without a separate drain line shall be provided under the coils on which condensate will occur. Such pan shall be equipped with a water-level detection device that will shut off the equipment served prior to overflow of the pan. The auxiliary drain pan shall be constructed in accordance with Item 1 of this section. 307.2.4 Traps. Since our initial meeting in early August I have finally completed the checklist of all items requested by your department for donation of the building. The process took far longer and was complicated by poor response from the utility companies. Columbia Gas necessitated up to 15 phone calls and repeated written requests to discontinue service. I have summarized and addressed every point requested on the Acquired Structure Checklist. 1. Two copies of the waiver of liability and authorization to burn form have been signed by my mother and father, both property owners (mom & dad). 2. A copy of the property deed is attached. 3. Statement from Nationwide Insurance informing that there is no insurance. 4. Letter from property owners (mom & dad) indicating there is no insurance on the structure. 5. Utility company disconnection a. AEP letter dated 09/01/2006, states that the energy service is de-energized. b. Columbia Gas, will not provide a letter stating line is disconnected; however, as of 11/30/2006 the meter has been removed and the line has been shut off at the street. They can confirm this by calling (800) 543-8911 and referring to reference # 500113401 c. Ameritech, phone service has been disconnected as of 09/21/2006 and attached lines removed from building. They will not issue a confirmation letter; however, this can be confirmed by calling (800) 727-2273 and referring to order # D165942369 and ticket # 0425389. 6. Asbestos inspection was performed on 07/27/2006 by Ohio Boris Karanfilov, M.D. Technical Services Inc. Building was found to have no 750 Mt. Carmel Mall, site 240 asbestos products. OBC 1802.2.1 calls for a soil report if the soil is questionable. In this case the soil in or near the pond is questionable. Wall Section B on Sheet A-6.1 does not dimension the footing. There is nothing to keep the "retaining wall"from tilting into the pond. The fill is called `free draining compacted aggregate" but there is no drainage the shown. It is likely that the pond water will drain back under the slab which presents a freezing expansion problem. Provide soil analysis for footing bearing and footing dimensions. Item 2. Note 19 on sheet A-1 calls for 3 - 9 Y4"LVLs to bear on 3 - 2X4's (post) at either end. This means that the third LVL would be bearing on air outside the face of the 2X4 wall, (wall section A sheet A-6.3). Two deeper LVLs could be used for the same load capacity. Item 3. Sheet SD-2 has a stair and a ramp; both are required to have handrails on both sides, see OBC 1009.11 and 1010.8 Item 4. General Comments • General Notes are to be provided as given in the Division of Engineering Administrative Policy 95-011. This information is available electronically on our website www.dublin.oh.us. General Notes have been added to the plans. The set submitted does not have general notes. Please keep in mind if you do this as teo sets you will need the general notes as part of each set. I would suggest you keep it as one set and add the necessary information. Attached is a checklist for that indicates what should be on the various pages. • Provide additional spot grades for the handicap spaces to ensure that no grades are greater than 2% in any direction. The spot grades for the handicapped spots have been added to the plans. Spotgrades are not shown on the plans submitted. • Add the following note above the City of Dublin signatures: Signatures below signify only concurrence with the general purposes and general location of the project. All technical details remain the responsibility of the Engineer preparing the plans. The note for signatures has been added. A note has been added for City of Columbus, but not for Dublin. Please change to read Dublin. • OEPA is now requiring that a PTI be issued before any construction of the sanitary sewer system can be completed. The plans may also need to be reviewed and signed by the City of Columbus. Contacts with these entities should be made concurrently with the City of Dublin review. This project is limited to the installation of a new 8" service lateral from the existing building to an existing 8" sanitary main line. As per the City of Dublin, the lateral is temporary until such time the new sanitary sewer for the Tuttle Road Extension is constructed. At that this property will be tributary to that sanitary sewer and will be connected to it. It is our understanding that PTI's are not need for service laterals. OEPA has determined that if it is a 6" service to one building, it is a service. All other configurations need to be submitted to them. Since this is an 8" with manholes, therefore it needs to be reviewed by OEPA. If the slope can work, you have the option of using a 6" with cleanouts which is considered a service by OEPA. • Architectural plans and civil plans show different things happening at the site. These will need to match each other. The building and civil plans have been corrected to match. Corrected plans were not submitted. Mr. Steven Bell September 16, 2005 Page 2 • There are existing protected trees on site, i.e. trees over 6" DBH. Conformance with the tree preservation ordinance is required. A tree survey, tree preservation plan and possibly a tree replacement plan needs to be submitted. Trees need to be protected with fencing prior to any construction activity, including demolition. A tree removal permit will need to be procured prior to the removal of any protected tree. • City staff has determined that the submitted drawings constitute a significant alteration to the existing building. According to City Code, expansion or alteration over 25% triggers minimum landscape compliance with the current landscape code. • Street trees need to be installed along Avery Road. The minimum caliper requirement for street trees was recently changed to 2.5" and I believe the approved species is Sweetgum. The city forester will need to be contacted prior to ordering and installing the trees. • The parcels surrounding this property are zoned R1-B (residential). According to the code, a six-foot high mound, planting, hedge, fence or wall needs to be installed along the entire property line. Also, a tree planting requirement of one per 40-feet is needed along the property and within 6-feet of the property line. Tree selections must be made from group A or B of City Code. Since the owner of the property to the east and south is the applicant, these areas do not need to meet with requirement at this time. However, a note needs to be added to the plans that will require the property owner of this medical building to meet the property perimeter requirements if ownership of the adjacent property is transferred. • The interior landscape requirement is deficient by 557 square feet. The parking lot peninsulas located near the entry may be used to help fulfill this requirement if a deciduous tree is added as required. A total of three deciduous trees are required within the parking area, and must be selected from Group A or B in the code. • The vehicular use area screening and tree planting requirement is deficient. Please revise drawings to show a 3.5-foot high, 100% opaque planting or mound along the north, west and south side of the parking area. This measurement is taken from the grade of the vehicular use area closest to the screening. The tree planting requirement is one deciduous tree from Group A or B every forty feet. • Tree plantings equal to 2.6 caliper inches needs to be added around the building to fulfill the additional site landscaping requirements. At least two deciduous or ornamental trees should be used around the building. • Please screen all service structures including transformers, heating and cooling units, etc. as required by code. • The drawing scale used for the landscape plan is unusual (1/32"=1'). However, a permit to build (even the foundation) cannot be issued until the plans are approved for various City of Dublin code elements. Those elements include evidence that the plans comply with required landscaping, and storm water management. Therefore, if the landscape plan, and storm water calculations requested in that email are not promptly submitted, any permit request will surely be denied, because the information is incomplete.
